Registration

A Rider Registration Form and Participant Release of Liability, Waiver, Indemnification, and Consent to Medical Attention are required for all registered riders, whether paid or riding without charge.  These forms can be completed in one of two ways.

Online:

Registration and Release forms may be completed and signed electronically (in compliance with the Electronic Signature in Global and National Commerce Act, the “E-Sign Act”) using Bike the Bend’s online registration and payment system. Payment is made by credit card automatically through a highly security encrypted system. We cannot see nor do we have acccess to your credit card information. 

To use the online registration system a computer with Internet access and Adobe Acrobat Reader, and a valid e-mail address is needed. We will email your registration confirmation and a copy of your signed Release to you for your records. You will also receive a credit card receipt via email.

The registration deadline to receive a rider T-shirt is May 1, 2013, but you can register online through Wednesday, May 15, 2013. 

Paper Registration:

 

After February 20, 2013, Registration and Release forms may be downloaded below, or available by calling Bike the Bend (574-204-2458) during business hours.  Forms will also be available at the Bike the Bend booth at Farmer’s Market on March 23, April 13 and May 4, 2013 and during Bike the Bend Packet Pickup on Saturday, May 18 (Times and Location will be announced).  Cash, checks and credit cards will be accepted.
 
Registrations and Release forms must be received by May 1 to guarantee your rider shirt.
